સ્વાદુપિંડનો રોગ / પેનક્રિયાટાઇટિસ શું હોય છે?

સ્વાદુપિંડનો રોગ એટલે કે પેનક્રિયાટાઇટિસ,  એક એવી સ્થિતિ છે જેમાં પેટમાં તીવ્ર દુખાવો કરી શકે છે.

સ્વાદુપિંડ એ એક અંગ છે જે એવા હોર્મોન્સ અને રસ બનાવે છે જે ખોરાક ને તોડવામાં મદદ કરે છે. પેનક્રિયાટાઇટિસ નો અર્થ એ છે કે જ્યારે આ અંગમાં સોજો આવે છે અથવા બળતરા થાય છે.

મોટાભાગના લોકો કોઈપણ સ્થાયી અસર વિના પેનક્રિયાટાઇટિસનું નિયંત્રણ કરે છે. પરંતુ અમુક લોકો ખૂબ જ માંદા પડે છે.

પેનક્રિયાટાઇટિસ થવાનું કારણ શું છે?

પેનક્રિયાટાઇટિસ થાવાના ઘણા કારણો છે. પરંતુ, મોટાભાગના કિસ્સાઓમાં પિત્તાશય અથવા દારૂના દુરૂપયોગને કારણે થાય છે:

  • “પિત્તાશય ની પાથરી / ગોલસ્ટોન્સ” (Gallstones)- ગોલસ્ટોન્સ એ સખત ગઠ્ઠો છે જે પિત્તાશય કહેવાતા અંગની અંદર બનતું હોય છે. પેનક્રિયાટાઇટિસ અને ગોલસ્ટોન્સ બંને એક જ નળીમાં ઉતરે છે. જો તે નળી એક પિત્ત-પથ્થર / ગોલસ્ટોન્સ થી ભરાઈ જાય, તો બેઉ માંથી કોઈ પણ અવયવો કોઈ પણ રસ ડ્રેઇન કરી શકશે નહીં. જ્યારે એવું થાય છે થાય છે, ત્યારે બંને અવયવોમાંથી પ્રવાહી પાછી આવે છે, જેનાથી પીડા થઈ શકે છે.
  • આલ્કોહોલનો / દારૂનો અતિરેક – જે લોકો ખૂબ લાંબા સમય સુધી અતિશય આલ્કોહોલ / દારૂ પીતા હોય છે, તેઓને કેટલીકવાર આલ્કોહોલથી સંબંધિત પેનક્રિયાટાઇટિસનો રોગ થાય છે. સામાન્ય રીતે, આ પ્રકાર ના પેનક્રિયાટાઇટિસ માં જે લોકો ઘણું દારૂ પીધા પછી, પીવાનું બંધ કર્યું હોય એના 1 થી 3 દિવસ પછી અચાનક પીડા અનુભવવાનું શરૂ કરે છે. તેમને સામાન્ય રીતે ઉબકા અને ઉલ્ટી પણ થાય છે.
પેનક્રિયાટાઇટિસ માટે કોઈ ખાસ પરીક્ષણ એટલે કે ટેસ્ટ ની જરૂર હોય છે?

એવા અમુક રક્ત પરીક્ષણો છે જે તમારા ડૉક્ટરને પેનક્રિયાટાઇટિસ હોય થયું છે કે નહીં એ જણાવી શકે છે. એ પણ શક્ય છે કે તમારા ડૉક્ટર તમારા  પૅટ નો સોજો તથા પેટમાં દુખાવો / પીડા, શું પેનક્રિયાટાઇટિસ અથવા બીજી કોઈ શારીરિક સમસ્યા ને કારણે ને થાય છે એ તપાસવા માટે તમારા પેટનું “સીટી સ્કેન” કહેવાતા એક વિશેષ પ્રકારનો એક્સ-રે કરાવવા કેહેશે.

પેનક્રિયાટાઇટિસનો ની સારવાર કેવી રીતે કરવામાં આવે છે?

સામાન્ય રીતે, પેનક્રિયાટાઇટિસનો ઉપચાર હોસ્પિટલમાં કરવામાં આવે છે. ત્યાં, તમારા ડૉક્ટર તમને પ્રવાહી અને પીડા ની દવાઓ આપી શકે છે જેથી તમને સારું લાગે. જો તમે ખાઈ શકતા ન હોવ, તો તેઓ તમને નળી દ્વારા ખોરાક આપી શકે છે.

પેનક્રિયાટાઇટિસનો રોગ ધરાવતા અમુક લોકોને ચેપ લાગે છે, જેનો ઉપચાર એન્ટીબાયોટીક્સથી કરી શકાય છે. પેનક્રિયાટાઇટિસના કારણે થતી અન્ય સંભવિત સમસ્યાઓ એ સ્વાદુપિંડ અંગની સંપૂર્ણ રીતે ખરાબ થઇ ને બંધ થઇ જવું અથવા તો એની આસપાસ પ્રવાહી બિલ્ડ-અપ (ભરાઈ જવું) થઇ જવું હોઈ શકે છે છે. પેનક્રિયાટાઇટિસની આસપાસ ફ્લુઇડ બિલ્ડ-અપ ઘણીવાર તેની જાતે જ જતું રહે છે જાય છે, પરંતુ કેટલીકવાર તેને શસ્ત્રક્રિયા દ્વારા ડ્રેઇન કરવાની અથવા તેની સારવાર કરવાની જરૂર પડે છે. સામાન્ય રીતે, સ્વાદુપિંડ અંગની નકામું થઇ જવાની સ્થિતિ ને ડોકટરોની ટીમ દ્વારા નિયંત્રિત કરવામાં આવે છે.

ઉપચારનો બીજો મહત્વપૂર્ણ ભાગ એ છે કે પેનક્રિયાટાઇટિસ થવા ના કારણોથી છૂટકારો મેળવવાનો છે. જો તમને પેનક્રિયાટાઇટિસ પિત્તાશય ની પાથરી / ગોલસ્ટોન્સ ના લીધે થાય છે, તો તમારા ડૉક્ટર ને ગોલસ્ટોન્સ ની સારવાર પણ કરવાની જરૂર પડી શકે છે. જે લોકો નો આલ્કોહોલના ઉપયોગથી પેનક્રિયાટાઇટિસનો રોગ થયો હોય એ લોકોએ ફરીથી પેનક્રિયાટાઇટિસનો રોગ થતો અટકાવવા માટે આલ્કોહોલ છોડી દેવાનું શીખવું અત્યંત આવશ્યક છે.

अग्नाशयशोथ या पेंक्रियाटायटीस (Pancreatitis) की बीमारी में क्या होता है?

अग्नाशयशोथ या पेंक्रियाटायटीस, एक ऐसी स्थिति है जिसमें गंभीर पेट दर्द हो सकता है। अग्न्याशय एक अंग है जो हार्मोन और पाचक रस बनाता, जिस्से जो भोजन को तोड़ कर हज़म करने में पाचन तंत्र को मदद होती है। में जब इस अंग में जलन या सूजन हो जाती है, इसे अग्नाशयशोथ या पेंक्रियाटायटीस कहता हैं। ज्यादातर लोगों का अग्नाशयशोथ, बिना किसी भी दीर्घावधि दुशप्रभाव, ठीक हो जाता है।

अग्नाशयशोथ क्या कारण से हो सकता है?

अग्नाशयशोथ के कई कारणों से होता हैं। लेकिन ज्यादातर क़िस्सों में पित्त पथरी या शराब के दुरुपयोग जैसे कारण होते हैं:

  • पित्ताशय की थैली – पित्ताशय की पथरी ऐसी कठिन गांठ होती है जो, पित्ताशय की थैली नामक एक अंग के अंदर बनती है। अग्न्याशय और पित्ताशय की थैली, दोनों ही अंग एक हाई ट्यूब या नाली में ख़ाली होते हैं। यदि उस नली में पित्त पथरी हो जाती है, तो डोनो में से कोई भी अंग आपने रस निकाल नहीं सकता है।

जब ऐसा होता है, तो दोनों अंगों के तरल पदार्थ वापस लौट के उनके अंदर हो भर जाते हैं। इससे तविर दर्द हो सकता है।

  • शराब का दुरुपयोग – जो लोग बहुत लंबे समय, तक बहुत अधिक शराब पीते हैं, उन्हें कभी-कभी शराब से संबंधित अग्नाशयशोथ हो जाता है। अग्नाशयशोथ के इस रूप वाले लोगों को, आमतौर पर या तो बहुत अधिक शराब पीने के बाद या फिर अचानक से शराब पीने का बंध करने के 1 से 3 दिन बाद दर्द महसूस होने लगता है। आमतौर पर, उन्हें मतली और उल्टी भी होती है।
अग्नाशयशोथ के लिए किसी विशेष परीक्षण यानी के टेस्ट की आवश्यकता है?

कुछ रक्त परीक्षण हैं जो आपके डॉक्टर को अग्नाशयशोथ होने का अंदेशा देते हैं। “क्या आपके पेट में दर्द अग्नाशयशोथ या अन्य स्थितियों के कारण हो रहा है?”, यह पता लगाने के लिए आपके डॉक्टर आपके पेट का एक विशेष प्रकार के एक्स-रे करवाने को कह सकते हैं, जिसे “सीटी स्कैन” कहते हैं।

एक अन्य महत्वपूर्ण उपचार होता है कि, अग्नाशयशोथ होने के कारणों से छुटकारा पाना। यदि आपके पित्ताशय की पथरी के कारण अग्नाशयशोथ होता है, तो आपके डॉक्टर को भी उनका इलाज करने की आवश्यकता हो सकती है। शराब के उपयोग से अग्नाशयशोथ वाले लोगों को फिर से अग्नाशयशोथ ना हो इस लिए शराब से छुटकारा पाने के लिए स्वयं कोशिश करनी चाहिए।

What is pancreatitis?
  • It is a condition that may cause severe belly pain.

    The pancreas is an organ that produces hormones and juices that help break down food. When this organ gets irritated or swollen, this condition is known as Pancreatitis. The pancreas can suffer from a variety of diseases.

    Most people recover from it without any long-lasting effects if adequate pancreatitis treatment is provided. However, some people get very sick.

What causes pancreatitis?

Is there a test for this condition?
There are a number of blood tests which will help your doctor find out if you have got this problem & decide pancreatitis treatment. it is also possible that your doctor will order a special kind of X-ray known as a “CT scan” of your belly to examine if belly pain is due to the condition in question.
How is pancreatitis treatment performed?

Pancreatitis treatment is performed in hospital. There, your doctor can provide you with fluids and pain medicines to help you feel better. If you can’t eat, they can provide you with food through a tube.

Some people may get an infection, which might be treated with antibiotics. Alternative possible issues caused are fluid build-up around the exocrine gland or organ failure. Fluid build-up around the pancreas usually goes away on its own, however, if it does not resolve then it must be drained or treated with surgery. Organ failure is sometimes handled by a team of doctors in medical care.

Another vital part of pancreatitis treatment is to get rid of the reason for the this condition. If your pancreatitis is caused by gallstones, your doctor may need to treat it, too. What is Alcoholic pancreatitis ?
  • Pancreatitis means swelling of pancreas. The swelling that results from alcohol ingestion is called alcoholic pancreatitis. Alcoholic pancreatitis is more common in men, which may result from a tendency for males to drink more rather than a gender-based difference in susceptibility . The peak age for presentation of alcoholic pancreatitis is uniformly 40 to 60 years.
What are the Effects of Alcohol on the Pancreas?
Heavy alcohol consumption Acute pancreatitis
  • Severe abdominal pain
  • Swollen and tender abdomen
  • Back pain
  • Jaundice (yellowing of the skin, whites of the eyes, and mucous membranes)
  • Low-grade fever
  • Nausea or vomiting
  • Loss of appetite
  • Increased heart rate
  • Low blood pressure
Repeated cases of acute pancreatitis cause irreversible damage to the pancreas, leading to chronic pancreatitis.
How does alcohol cause acute pancreatitis?
It is not yet understood exactly how alcohol causes acute pancreatitis. One theory is that the alcohol molecules interfere with the cells of the pancreas, stopping them working properly. The other theory proposes increased thickness of pancreas fluid which blocks the pancreas ducts.
But, whatever the cause, there is a clear link between drinking alcohol and acute pancreatitis.

What is a gallstone pancreatitis?
  • Pancreatitis is a disease that causes inflammation and pain in your pancreas. The pancreas is a small organ that makes fluids and enzymes to break down the food you eat. Sometimes a gallstone(stone in the gallbladder) can block your pancreatic duct and cause pancreatitis. ,This is known as gallstone pancreatitis.
What are the symptoms of gallstone pancreatitis?
Severe pain is the most common symptom of gallstone pancreatitis, but it is not the only symptom. You might also have:
  • Pain that feels sharp or a “squeezing” in your left upper abdomen or in your back
  • Pain that radiates from the original site up to the shoulder or chest
  • Chills
  • Fever
  • Jaundice
  • Nausea
  • Vomiting
What causes gallstone pancreatitis?
Eighty percent of cases of pancreatitis are caused by alcohol or gallstones . Gallstones are the single most common cause of acute pancreatitis. Alcohol is the single most common cause of chronic pancreatitis.
Gallstones form in your gallbladder. But in cases of gallstone pancreatitis, the stone leaves the gallbladder. It blocks the opening from the pancreas to the first part of the small intestine (duodenum). This causes a backup of fluid that can travel up both the bile duct and the pancreatic duct.
How can I prevent gallstone pancreatitis?
It’s not possible to fully prevent gallstone pancreatitis. This is because it’s not possible to fully prevent gallstones from forming. But you can take steps to reduce your gallstone risk.
Eat a healthy diet, stay at a healthy weight, keep your cholesterol levels within a normal range, and manage diabetes if you have it.
If you are taking a medicine that contains estrogen, ask your healthcare provider how this might add to your risk of developing gallstones.
If you already have a gallbladder stone then consult your doctor regarding your risk of developing pancreatitis. Having multiple small stones, increases the risk of pancreatitis. In such scenario, the doctor will advise surgery to remove gallbladder.
How is gallstone pancreatitis treated?
Pancreatitis is best treated initially by avoiding any intake of liquids and solids until the inflammation subsides. Intravenous delivery of fluids is usually all that’s required if the inflammation is modest and symptoms resolve in a few days.
 
Severe inflammation, persistent pain or fevers suggest severe pancreatitis and ongoing inflammation. Intravenous delivery of nutrients would be started if oral intake cannot be restarted within approximately 5-7 days. Severe nausea and vomiting are treated initially by relieving the stomach of fluid by use of a nasogastric tube and with antinausea medications.
 
Pain therapies may be administered by intravenous until oral treatments and food intake can resume.
 
Stones that cause gallstone pancreatitis may pass out of the duct without intervention or may require endoscopic &/or surgical removal.
 
In cases of infected pancreatic tissue, or a condition called pancreatic necrosis (dead tissue) occurs, antibiotics may be used to control or prevent infection.

What is Hereditary Pancreatitis?
  • Hereditary pancreatitis(HP) is a genetic condition characterized by recurrent episodes of inflammation of the pancreas (pancreatitis). The pancreas produces enzymes that help digest food, and it also produces insulin, a hormone that controls blood sugar levels in the body.
What are the symptoms of hereditary pancreatitis?
The first symptoms of hereditary pancreatitis usually occur in children or early adults. Symptoms may include:
  • abdominal pain
  • nausea
  • vomiting
  • fever
These symptoms are acute, meaning they occur suddenly, and may last for several days. Later in life, these acute episodes begin to recur, meaning they begin to happen more frequently. This begins the development of chronic pancreatitis, in which the pancreas is continually in stress from inflammation.
What causes hereditary pancreatitis?
HP is a genetic condition. This means that the risk of pancreatitis and pancreatic cancer can be passed from generation to generation in a family. The gene most commonly associated with HP is called PRSS1. A mutation (alteration) in the PRSS1 gene gives a person an increased risk of pancreatitis and pancreatic cancer. Mutations in other genes, including SPINK1, CTRC, CASR, and CFTR, have also been linked to HP.
How is Hereditery Pancreatitis diagnosed?
In cases of hereditary pancreatitis, physicians use many of the same tests performed for acute and chronic pancreatitis such as blood tests (Amylase, Lipase), Ultrasound or CT Scan. Once pancreatitis is established, a final diagnosis of hereditary pancreatitis is made when the genetic mutation PRSS1 is found.
The diagnosis of HP is considered when 2 or more close family members, meaning parents, siblings, or children, in at least 2 generations have recurrent pancreatitis. Genetic testing is sometimes considered for people who develop recurrent pancreatitis at a young age. Genetic testing is available for mutations in the PRSS1, SPINK1, CTRC, CASR, and CFTR genes. Talking with a genetic counselor is recommended before having genetic testing.
What is the treatment for hereditary pancreatitis?
The most fundamental treatments that a patient can do for pancreatitis are:
– alcohol cessation alcohol is a known risk factor in the development of pancreatitis
– smoking cessation tobacco products are a known risk factor for many conditions
– diet, obesity and high fat foods need to be controlled
Other treatment methods may also include:
  • pain medications
  • enzyme replacement therapy
  • Endoscopic or surgical procedure to remove stricture or stone in chronic pancreatitis
