Diagnostic value of barium oesophagram compared to pH-impedance monitoring in the detection of paediatric gastro-oesophageal reflux.DOI 10.1136archdischild-2025-328994

Acid reflux is common in children. It can cause symptoms like vomiting, chest discomfort, or poor feeding. Doctors use different tests to confirm it. Two common tests are the barium swallow and pH-impedance monitoring.

A recent study looked at how accurate the barium test is. It included 90 children who had both tests done. The goal was to see if the barium test can reliably detect reflux.

The results were surprising.

The barium test showed reflux in about 4 out of 10 children. The pH test, which is more accurate, showed reflux in only about 2 out of 10 children. This means the barium test may suggest reflux even when it is not truly present.

The study also found no strong link between the two tests. A child could have a positive barium test but still not have real acid reflux on detailed testing.

There was one exception. Children with a hiatal hernia were more likely to show reflux on the barium test. Even then, this did not always match the pH test results.

What does this mean for parents?

The barium test is useful to look at the structure of the food pipe and stomach. It can detect blockages or abnormalities. It is not reliable for confirming acid reflux.

Doctors often rely on pH monitoring or other tests for a clear diagnosis. These tests measure acid levels over time and give better answers.

If your child has reflux symptoms, do not depend on a single test result. A proper evaluation is needed.

Key takeaways:

⦿ Barium test alone may not confirm reflux
⦿ pH monitoring gives more accurate results
⦿ Symptoms and clinical evaluation are important
⦿ Always discuss results with your doctor

Early and correct diagnosis helps your child get the right treatment.
