In short
After a bleeding or burst ulcer, the follow-up decides whether the ulcer returns. It rests on three jobs: remove the cause, prove healing, and rule out cancer.
Check that H. pylori has gone, settle painkillers, aspirin and blood thinners with a plan, and repeat the scope for a stomach ulcer.
Black stool, vomit with blood, or sudden severe belly pain after you go home needs an emergency department the same day.
The full care plan is on the peptic ulcer treatment in Ahmedabad page. This post explains what follow-up is needed once the bleed or the hole has been treated.
After a bleeding ulcer is treated, the next three days carry the highest risk of bleeding again. This is why most people stay in hospital for a while.
If bleeding returns, the next step is usually a repeat scope. If that fails, a radiologist can block the bleeding vessel through a thin tube. Surgery is kept for the few who still bleed. The first treatment is described in how a scope stops the bleed.
Whether the ulcer bled, burst or blocked the stomach, follow-up has three jobs. Remove the cause, prove the ulcer has healed, and rule out cancer.
| What happened | What is done next | Why it matters |
|---|---|---|
| Bleeding ulcer | Acid-blocking medicine, H. pylori test and treatment, painkillers stopped or covered | Stops the bleed from returning |
| Hole in the wall (perforation) | H. pylori treatment after the repair, plus a scope for a stomach hole | Without it, the ulcer often comes back |
| Blocked stomach outlet | Feeding and salt correction, then a scope with samples | A growth can look like a scar, so it must be ruled out |
| Ulcer eating into a nearby organ | A CT scan and a surgical opinion | Pain that turns constant and goes to the back is the clue |
The ulcer is rarely the danger by itself. Age, other illnesses and delay in reaching care decide how a person fares. That is why a clear plan for follow-up matters as much as the first treatment. A hole in the wall is covered in perforated peptic ulcer surgery.
Treating the H. pylori germ is what stops most ulcers from returning. Do not assume it worked. Check it once the course is over.
If the germ is still there, a different antibiotic combination is used. If an ulcer returns after a clean test, the search turns to hidden painkiller use or a rare acid-making growth. The testing steps are in stomach pain: is it an ulcer.
Stop painkiller tablets that harm the stomach. Do not stop heart-saving aspirin or blood thinners on your own, because stopping them can be more dangerous.
| Medicine | Usual plan | Who decides |
|---|---|---|
| Painkillers such as diclofenac or ibuprofen | Stop. Ask for a safer option for joint pain | Your surgeon and family doctor |
| Aspirin for a stent or heart attack | Restart within a few days, with acid cover | Your heart doctor and surgeon together |
| Aspirin taken only as a precaution | Often stopped for good | Reviewed one case at a time |
| Blood thinners for the heart rhythm or a valve | Restart on a set date once bleeding is controlled | Your heart doctor and surgeon together |
Every medicine that is held back needs a restart date and a name against it. Leaving a heart patient off blood thinners by default is a hidden risk. Many herbal balms and powders also contain painkillers, so mention them all.
A stomach ulcer can hide a cancer, even if it looks harmless and begins to heal. A repeat scope with samples is how this is excluded.
This one visit is the most skipped step in follow-up. People feel better and do not return. Please keep the appointment even if you feel well.
Most ulcers whose cause is removed need acid tablets for a limited time. Some people need them long term, for a clear reason.
Long-term acid tablets are largely safe for people who need them. Do not stop them in fear, and do not carry on for years without a reason.
Most ulcer problems today are treated without removing part of the stomach. If you did have stomach surgery, some late problems are expected and can be managed.
| Problem | How it shows | What helps |
|---|---|---|
| Early dumping | Cramps, loose stool and flushing within half an hour of a meal | Small, frequent meals; fluids taken apart from solids |
| Late dumping | Sweating and shakiness one to three hours after a meal | Fewer sugary foods; a snack that holds sugar steady |
| Bile reflux | Burning pain with green-yellow vomit that does not relieve the pain | Medicines first; a bypass operation if it persists |
| Low iron, B12 or calcium | Tiredness, tingling feet, weak bones | A yearly blood check and supplements |
After the whole stomach is removed, vitamin B12 must be given for life. Years after any partial stomach surgery, new pain or vomiting is a reason to scope the remaining stomach.
Some signs mean the ulcer has bled again or burst. These need an emergency department the same day, not a clinic appointment next week.
Pain that turns constant and spreads to the back is not an emergency in itself. It does need a call to your surgeon within days.
A breath or stool test, done at least four weeks after the antibiotics. Stop acid tablets two weeks before, if safe for you.
Some stomach ulcers hide a cancer, even when they begin to heal. A scope after six to eight weeks, with samples, makes sure none is missed.
Often yes, within a few days, with acid-blocking cover. Do not stop or restart it alone. Your heart doctor and surgeon decide together.
Painkillers such as diclofenac and ibuprofen are usually stopped. Ask your doctor for a safer option for joint pain.
A short course if the cause is removed. Longer if you must stay on aspirin or blood thinners, or if the ulcer was serious or returned.
Black stool, vomit with blood, fainting, or sudden severe belly pain with a hard belly. Go to an emergency department the same day.
