In short
A perforated ulcer is a hole right through the wall of the stomach or the duodenum. The pain starts suddenly and is severe.
Drip fluids, antibiotics, a stomach tube and an acid-blocking drip come first. The leak is then closed within hours, not days.
The usual repair stitches the hole shut and lays a tongue of omental fat over it. The whole belly is then washed out.
A perforation is the emergency end of peptic ulcer disease. The full picture, from ulcers that bleed or narrow the stomach outlet to the planned operations for them, is on the main peptic ulcer and gastric outlet obstruction surgery page.
An ulcer is a raw break in the lining of the stomach or the duodenum, the first part of the small bowel. If it eats right through the wall, a hole forms and stomach juice leaks into the belly. The pain begins suddenly, high in the belly, and spreads within minutes. The belly wall goes hard, like a board. Most people can name the hour it began.
Sudden severe pain high in the belly that spreads, with a hard belly wall, is treated as a perforation until tests say otherwise.
A perforation runs through three stages. First the leaked acid irritates the lining of the belly. Then comes a quiet spell, often six to twelve hours in: the pain eases and the patient looks better, though nothing has healed. The third stage is infection across the whole belly, and then shock. Many patients reach a hospital during that quiet spell, and some are sent home. Those lost hours change the outcome more than anything a surgeon does later.
| Test | What it shows | What to remember |
|---|---|---|
| Chest X-ray, standing | Free gas under the diaphragm. | A normal film does not rule out a hole. |
| CT scan of the belly | Where the hole is, and how much has leaked. | The test of choice when the patient is steady and the diagnosis unclear. |
| Contrast swallow study | Whether the hole still leaks or has sealed. | Used only when treatment without an operation is weighed. |
A normal chest X-ray does not rule out a perforated ulcer. If the belly is rigid, the next step is a CT scan, not reassurance.
Stopping the leak is urgent, but not instant. A short, hard push of resuscitation makes the operation safer to withstand.
| Step | Why it is done |
|---|---|
| Drip fluids | Restore blood pressure and blood supply to the organs. |
| Antibiotics into the vein | Cover the gut germs that leaked out. An antifungal is added in late or frail patients. |
| Tube through the nose | Empties the stomach, so less juice keeps leaking. |
| Catheter in the bladder | Urine output shows whether the fluids are working. |
| Acid-blocking drip | Cuts the acid still leaking, and helps healing. |
| Theatre within hours | Each extra hour of delay counts against a sick patient. |
The order is resuscitate fast, then operate fast. A septic patient waiting for perfect numbers drifts down, not up. Belly fluid is sent for culture during the operation, so antibiotics can be narrowed later.
Three things decide risk far more than the kind of repair: low blood pressure on arrival, symptoms running beyond a day, and other serious illness. A patient with all three needs intensive care and a frank talk with the family. Steroids, diabetes and old age also hide the signs, so the belly can feel soft while a hole sits wide open. In these patients the blood tests and the scan are trusted over how the belly feels.
The standard repair has been done for decades and still holds its place. Three stitches pass through the full thickness of the wall, either side of the hole. The omentum, the fatty apron inside the belly, is drawn across as a living flap, and the stitches are tied over it.
The omentum seals the hole from outside. It is not pushed into it. The stitches are tied snug, never tight enough to strangle the fat.
The surgeon then hunts for a second hole, washes out the belly and pelvis until the fluid runs clear, and tests the repair for leaks. A drain goes in only for a large or doubtful repair, or a very dirty belly.
| Points towards keyhole | Points towards an open operation |
|---|---|
| Blood pressure steady. | Blood pressure low, or drugs holding it up. |
| Small hole. | Large hole, or edges soft and crumbling. |
| Reached hospital within a day. | Came late, infection already across the belly. |
| Leak held to one area. | Soiling in all four corners of the belly. |
| A trained team and the kit are ready. | Thin night cover, or no keyhole kit. |
Keyhole repair means less wound infection and less pain afterwards, with leak and safety much the same in suitable patients. Switching to an open operation for a poor view or a falling blood pressure is sound judgement, not failure. In many hospitals at night, a quick open repair is the right call.
Some holes cannot be patched. A very large hole in the duodenum, or edges too soft to hold a stitch, will tear out again. The choices then get bigger: a patch made from small-bowel wall, a tube placed into the duodenum to drain it, a bypass that closes off the stomach outlet, or removal of the lower stomach.
A very large or crumbling hole will not hold a patch. The operation then has to be bigger, chosen on what the patient can withstand.
A hole through a growth in the stomach is a separate problem, because a patch over a tumour will not heal. If the patient can take it, the tumour is removed. If not, the edges are biopsied, the hole patched, and the cancer plan made after recovery.
A small, carefully chosen group is treated without surgery: nothing by mouth, a stomach tube, drip fluids, antibiotics and an acid-blocking drip, with frequent review. All of these have to be true:
If there is no improvement within about half a day, the patient goes to theatre. Older patients fail this route more often. Where there is doubt, an operation is the safer answer.
Closing the hole stops the leak. It does nothing about why the ulcer formed. The old operation that cut the acid nerves has been abandoned, because tablets do that job better.
| Item | What is done |
|---|---|
| Helicobacter pylori | Tested for and treated. Clearing it is what stops the ulcer returning. |
| Proving it has gone | A breath or stool test, four weeks after the antibiotics and two weeks off acid tablets. |
| Painkillers | The anti-inflammatory tablet stops. Aspirin for the heart continues, under acid cover. |
| A stomach ulcer | Biopsied during the operation, and scoped again later to document healing. |
| Feeding and drains | Feeding restarts as the bowel wakes. Drains come out once the fluid runs clear. |
Every hole in the stomach has its edges biopsied during the operation. A perforated growth and a perforated ulcer look the same at midnight.
An ulcer still unhealed at the repeat endoscopy is biopsied again, not given one more month of tablets. Scarring at the stomach outlet can also appear months later, with vomiting of stale food; that sits on the peptic ulcer and gastric outlet obstruction page.
Within hours of reaching hospital, in most cases. Fluids and antibiotics run at the same time, not before.
Yes. Up to a quarter of perforations show no free gas on a plain film. If the belly is rigid, a CT scan follows.
Keyhole suits a steady patient who came early with a small hole. A late, dirty or unstable belly is safer with an open operation.
A hole in the stomach can be a hole through a growth. Both look alike during the operation, so the edges are always biopsied.
It can, if Helicobacter pylori is left untreated or the painkiller continues. Clearing the germ and stopping that tablet is what prevents it.
