In short
A bleeding ulcer is treated in a fixed order. Fluids and blood come first. Then comes a scope, with a clip or heat on the bleeding spot.
The scope is usually done within a day, once the body is steady. Rushing it does not save more lives.
Most bleeds stop at the scope. Strong acid control and treating H. pylori then keep the ulcer from bleeding again.
The full care plan is on the peptic ulcer treatment in Ahmedabad page. This post explains how a bleeding ulcer is stopped through a scope, and what happens before and after.
Blood from an ulcer can drain the body fast. Go to a hospital at once if you see any sign below.
Do not drive yourself. Do not wait to see if it settles. The sooner a team steadies you, the safer the scope will be. A different ulcer emergency, a hole in the wall, is explained in perforated peptic ulcer surgery.
Fluids and blood come first, and the scope second. A body that is not steady cannot take a scope safely.
| Step | What is done | Why |
|---|---|---|
| Drips and blood tests | Two large drips, fluids, and blood kept ready | Fills the body and keeps blood at hand |
| Blood transfusion | Given only when the blood count falls below a set low level | Too much blood raises pressure in the veins and can restart the bleed |
| Clotting check | Blood thinners are reversed if the bleed threatens life | Lets a clot form on the ulcer |
| Breathing | A breathing tube if vomiting is heavy or the person is drowsy | Keeps blood out of the lungs |
| Risk score | A score from blood tests and vital signs | Shows who must stay and who is at very low risk |
Many families expect that more blood is safer. It is not. Giving blood only below a set level has been shown to lower deaths and re-bleeding. People with heart disease are given blood a little earlier.
Some medicines help while you wait for the scope. None of them replaces it.
For most people the scope is done within a day, once the body is steady. A rushed scope is not safer.
A large trial compared a very early scope with a scope within a day. The very early scope did not save more lives. It may even do harm, because an unsteady person tolerates it poorly.
One group is different. A person who stays unwell despite fluids and blood is scoped once as steady as possible. A person at very low risk may be scoped as an outpatient.
The look of the ulcer base tells the doctor how likely it is to bleed again. That decides whether treatment is needed.
A stomach ulcer is also sampled with a biopsy. The report shows whether it is an ordinary ulcer.
No single tool is used alone. Adrenaline is paired with a clip or a heat probe, because adrenaline wears off.
Ulcers at the back of the first part of the small bowel lie close to a large artery. So do ulcers high on the inner curve of the stomach. These are the ones that most often bleed again. Every step carries a small risk, such as a hole in the wall or a bleed that continues. The team watches for both.
After a high-risk ulcer is treated, a strong acid-lowering drip runs for about three days. These are the days a re-bleed is most likely.
A repeat scope is the first answer to a re-bleed. Radiology comes next, and surgery is the last step.
Old operations that cut acid are no longer done. Tablets and H. pylori treatment do that work. Today surgery only stops the bleeding. Round-the-clock radiology is not available everywhere. An unsteady person with a repeat bleed may need transfer to a hospital with both radiology and surgery.
Do not stop heart-protecting aspirin on your own. After the ulcer is treated, it is usually continued with acid cover.
Finding and treating H. pylori is a major step against another bleed. It works for longer than any step taken during the bleed.
Some small bleeds stop on their own. No one can tell without a scope, so every suspected bleed needs a hospital.
Yes, once the body is steadied. Fluids, blood and breathing support are kept ready.
More blood is not safer. Giving blood only below a set level lowers deaths and repeat bleeding.
Mostly not. Most bleeds stop at the scope. Surgery is for a bleed that continues after repeat scope and radiology treatment.
If it protects your heart, usually yes, with an acid tablet. Decide with your heart doctor, never alone.
Yes, if H. pylori or painkillers continue. Treat H. pylori, avoid painkiller tablets and take acid medicine as advised.
