In short
Ulcer pain is a burning ache in the upper belly, but the pattern is a poor guide. A test shows whether an ulcer is there.
Bleeding, weight loss, anaemia, repeated vomiting or new pain after middle age needs a scope soon. Do not wait or take acid tablets first.
Younger people with none of these signs can start with a simple H. pylori breath or stool test. A stomach ulcer always needs biopsies.
The full care plan is on the peptic ulcer treatment in Ahmedabad page. This post explains the signs of an ulcer, the red flags, and the tests used to prove it.
An ulcer usually shows as a burning ache in the upper belly. The pattern is a weak clue, so a test decides.
| Feature | Duodenal ulcer (first part of small bowel) | Stomach ulcer |
|---|---|---|
| Food | Pain eases after a meal and returns a few hours later | Pain starts or worsens with a meal |
| Night | Often wakes you in the early hours | Less often |
| Weight | Usually steady, as eating gives relief | May fall, as you avoid food |
| Main cause | H. pylori germ, then painkiller tablets | Painkiller tablets, H. pylori, and rarely cancer |
Most people with upper belly pain do not have an ulcer. Many have functional dyspepsia or acid reflux. Some ulcers give no pain at all, mostly those caused by painkiller tablets. They show up first as a bleed or a hole. Older people also under-report pain.
Some signs mean an organic problem, sometimes a cancer, must be ruled out by a scope without delay.
Sudden, board-like belly pain with a rigid abdomen points to a hole in the wall. That emergency is covered in perforated peptic ulcer surgery. In India, where stomach cancer can appear at a younger age, keep the threshold for a scope low.
Without red flags, a simple H. pylori test can come first. With red flags, or in older people, the scope comes first.
| Your situation | First step | Why |
|---|---|---|
| Younger, upper belly pain, no red flag | H. pylori breath or stool test, then treat if positive | Works as well as a scope for symptoms, and spares a scope |
| Any red flag | Scope now | Finds ulcers and cancers early |
| New pain after middle age | Scope now | The chance of cancer rises with age |
| Months on acid tablets with partial relief | Scope, if red flags or older age | Relief does not prove a harmless cause |
One mistake is common. Acid tablets are started for a person who has a red flag. They hide the picture, delay a cancer diagnosis and spoil the H. pylori test that comes later.
The scope shows the ulcer, takes samples, and can stop a bleed in the same sitting. A stomach ulcer always needs samples.
The look of an ulcer never replaces a biopsy. A smooth edge is reassuring, but it is not proof.
Acid tablets and antibiotics can hide the germ. Stop them for the right period before the test, or the result may read falsely negative.
| Test | What it finds | Remember |
|---|---|---|
| Breath test | Active germ | Reliable. Needs a gap from acid tablets and antibiotics |
| Stool antigen test | Active germ | Works as well as the breath test, with the same gap |
| Test on a scope sample | Active germ | Quick at the scope. Can miss it after a bleed or on acid tablets |
| Blood antibody test | Past or present contact | Cannot show that the germ is gone |
Blood tests and scans look for complications and other causes. They do not replace a scope for the ulcer itself.
Work down the list of dangerous causes first: cancer, a hole, bleeding and the heart. Call the pain functional only after the scope and these tests are normal.
Many ulcers, ulcers far down the small bowel, or ulcers that keep returning call for a search beyond H. pylori and painkillers.
If the stomach cannot empty and vomiting continues, a scope is needed to check that the blockage is not a growth. Do not stop acid tablets on your own if you have had a bleed.
No. Ulcer pain looks like many other problems. A test or a scope is needed to be sure.
No. Younger people with no red flag can start with an H. pylori test. A scope is needed if any red flag is present.
Some stomach ulcers hide a cancer. Several samples and a repeat scope after healing make sure none is missed.
Acid tablets lower the germ count and can give a false negative. Stop them two weeks before, if safe for you.
Yes. Many people have functional dyspepsia or reflux. Other causes such as gallstones are checked as well.
No. The blood antibody stays positive after the germ is gone. Use a breath or stool test, four weeks after the course.
