In short
The stage describes three things. T is how deep the tumour has grown, N is the lymph nodes, and M is spread to distant places.
Stage is set twice. Scans and laparoscopy give a clinical stage before treatment. The removed stomach gives the final stage after surgery.
Cancer cells in the abdominal wash count as distant spread, even when nothing is seen. That makes it stage IV and changes the plan.
Stomach cancer reports use the TNM system, eighth edition. Surgeons also use the Japanese system. It gives each lymph node group around the stomach a number, called a station. The Japanese system guides how wide the node removal should be. Both systems arrive at the same T, N and M letters.
A small letter in front of the stage tells you when it was set. “c” means before treatment, from the scans and the keyhole look. “p” means after surgery, from the removed stomach. “yp” means after surgery that followed chemotherapy.
The stomach wall is made of layers. T tells you how many layers the tumour has crossed, and whether it has reached a nearby organ.
| T | What it means |
|---|---|
| Tis | Abnormal cells in the inner lining only. No invasion yet. |
| T1a | Into the thin inner layers of the lining. |
| T1b | Into the layer under the lining, called the submucosa. |
| T2 | Into the main muscle layer. |
| T3 | Through the muscle, into the tissue under the outer covering. |
| T4a | Through the outer covering of the stomach. |
| T4b | Into a nearby organ, such as the spleen, pancreas, liver, colon or diaphragm. |
The line between T1a and T1b matters for very early cancer. A T1a cancer may be removed through the endoscope. A T1b cancer usually needs an operation.
N tells you how many of the nearby lymph nodes contain cancer. The final answer comes from the pathologist, after the nodes are removed with the stomach.
| N | What it means |
|---|---|
| N0 | No lymph node holds cancer. |
| N1 | A small number of nodes hold cancer. |
| N2 | More nodes hold cancer. |
| N3a | Many nodes hold cancer. |
| N3b | A large number of nodes hold cancer. |
The N stage is only reliable when enough nodes are examined. This is one reason the operation removes the node groups around the stomach in one piece. Some nodes are too far away to count as N. Nodes beside the aorta, or along the root of the bowel’s vessels, count as distant spread. So do nodes behind the head of the pancreas, unless the tumour is in the lower stomach.
M0 means no distant spread. M1 means spread to places such as the liver, lungs, distant nodes or the abdominal lining, including cells in the wash.
The saline wash taken at the keyhole look has its own label. CY0 means no cancer cells in any wash. CY1 means cancer cells were found. CY1 counts as M1, even when no deposit can be seen. That is why the keyhole look with a wash is treated as part of staging, not an extra.
T, N and M are combined into one stage group. The table below is simplified. Your own report gives the exact letters.
| Stage | In plain words | What it usually means |
|---|---|---|
| 0 | Abnormal cells in the inner lining only. | The earliest form. The plan is set at the tumour board. |
| I | A tumour in the inner layers with no nodes. Or a slightly deeper tumour, or a small number of nodes. | Early. A T1a tumour with no nodes may be removed through the endoscope. Others need an operation. |
| II and III | Deeper growth, more nodes, or both. | Locally advanced. An operation with D2 node removal is the standard. The tumour board decides if chemotherapy comes first. |
| IV | Spread to distant organs, distant nodes or the abdominal lining, or cancer cells in the wash. | Medicines are the main treatment. An operation to remove the stomach is not the first step. |
The order of chemotherapy and surgery for each stage is explained on the chemotherapy before surgery page.
The clinical stage is a careful estimate made before treatment. The final stage comes from the pathologist, who examines the removed stomach and every node. It can come out higher or lower than the clinical stage.
When chemotherapy is given before surgery, the final stage carries the prefix “yp”. The same letters are used. But a yp stage is read differently from a stage set without chemotherapy, because the outlook differs, stage for stage.
ypT0 N0 means no cancer was left in the stomach or the nodes after chemotherapy. This complete response is a strong good sign.
The report may also give a regression grade. It describes how much of the tumour the chemotherapy destroyed. It is not part of TNM, but it adds to the picture.
A tumour at the join of the food pipe and the stomach is grouped by where its centre lies. This grouping decides which operation is planned.
| Type | Where the centre lies | Usual operation |
|---|---|---|
| Type I | Above the junction, in the food pipe. | Treated as a food pipe cancer, with removal of the food pipe. |
| Type II | Right at the junction. | Depends on how far it runs up the food pipe. It may be a food pipe operation, or an extended removal of the whole stomach. |
| Type III | Below the junction, in the stomach. | Treated as a stomach cancer. The whole stomach is removed, with D2 node removal. |
More on upper stomach and junction tumours is on the Siewert level page.
This usually happens when the laparoscopy wash shows cancer cells, or tiny deposits are seen on the lining. CT cannot show these.
Yes. The final stage comes from the removed stomach and nodes. It can be higher or lower than the stage before surgery.
No lymph node examined held cancer. It is reliable only when enough nodes were examined.
An operation is not the first step. In a few people with cells only in the wash, chemotherapy clears them and surgery becomes possible.
It is the final stage after chemotherapy and then surgery. It uses the same letters, but doctors read it differently.
