In short

Albendazole is a worm-killing tablet. It is given before surgery to weaken the parasite inside the cyst, and after surgery to clear any that were left or spilled.

It lowers the chance that the cyst comes back. It does not replace the operation for a large cyst.

The tablets are taken with a fatty meal, and blood tests are done to check the liver and blood counts while you take them.

The causes, tests and treatment choices for a hydatid cyst or a liver abscess are on our liver abscess and hydatid cyst page. This post explains one thing: what albendazole does around the operation.

What is albendazole and why is it used?

Albendazole is a medicine that kills tapeworm larvae. A hydatid cyst is a bag of such larvae, so the tablet acts on the cyst itself.

A hydatid cyst is caused by a tiny tapeworm that lives in dogs. People pick up the eggs from soil, water or raw vegetables. The eggs hatch in the gut and travel to the liver, where each one can grow into a fluid-filled cyst.

Inside the cyst are many young worms. If these escape into the belly during surgery, each can start a new cyst. Albendazole is used so that the worms inside are weak or dead before the cyst is opened.

Why is it given before the operation?

The tablets are started before surgery so that the worms inside the cyst are no longer alive when the surgeon handles it.

  • Safer handling. If the cyst leaks while it is removed, the spilled worms are less likely to grow.
  • Softer cyst. Some cysts shrink or soften, which can make the operation easier.
  • Less risk of a severe allergy. A cyst that leaks can set off a sudden allergic reaction. Pre-treatment, together with careful surgery, lowers this risk.

The usual plan is to start the tablets some days to a few weeks before the operation. Your surgeon sets the exact time from your scan and how soon surgery is needed. A cyst that is infected, or pressing on a bile duct, should not be left waiting for the tablets.

Why is it continued after the operation?

After surgery the tablets are continued, usually for one to three months, to clear any worms that may have been left behind.

Even a careful operation can leave a few microscopic worms behind. The aim of the course after surgery is to stop these from growing into a new cyst. This is the main reason hydatid disease comes back in some people.

How are the tablets taken?

The dose depends on your body weight and is split across the day. Take it with a meal that contains some fat, because the body absorbs it better then.

Your doctor writes the dose for you. Do not change it, and do not stop early because you feel well. Missing days can leave living worms in the cyst.

  • Take the tablets after food, with milk, ghee or a meal that has some fat.
  • Take them at the same times each day.
  • If you miss a dose, take the next one at the usual time. Do not double up.
  • Keep taking them until your surgeon says the course is over.

What side effects should you watch for?

Most people have no problem. The usual concern is a rise in liver tests, so a blood test is done at regular visits while you take the tablets.

  • Mild nausea, stomach upset or headache
  • Rise in liver enzymes, usually without symptoms
  • A fall in the blood count, which is why a blood test is repeated
  • Hair thinning, which settles after the course
  • Yellow eyes or dark urine, or a high fever, which need a call to us the same day

Pregnant women should not take albendazole, and a woman planning pregnancy should tell the doctor first. Tell us about any liver disease or other medicines you take.

Does albendazole alone treat a hydatid cyst?

For a small, simple cyst the tablets may be enough. A large cyst, an infected cyst or one pressing on the bile duct usually needs a procedure as well.

Tablets suit small cysts. They cannot remove a big cyst with a thick wall. The choice between tablets alone, a needle procedure and surgery depends on the size, the stage on scan, and where the cyst lies. Our liver abscess post explains how a liver collection is told apart from a hydatid cyst.

What happens after the course is over?

You will have blood tests and an ultrasound at planned visits for a few years, to make sure no new cyst is forming.

Most people return to normal life within weeks of the operation. A cyst that comes back is usually small and found early on a scan, when it is easier to treat. Wash vegetables well, drink safe water, wash hands, and keep pet dogs dewormed to lower the chance of catching the eggs again. For what the operation involves, see our post on liver operations.

Questions to ask your surgeon

Ask when to start, how long to continue, which tests to repeat, and what to do if a side effect appears.

  • When should I start albendazole, and for how long?
  • Which blood tests will I need, and how often?
  • Do I need a procedure or surgery as well as the tablets?
  • What follow-up scans will I need after the course?
  • Which symptoms mean I should call you at once?

Frequently asked questions

Can albendazole dissolve a hydatid cyst on its own?

Sometimes a small, simple cyst shrinks or settles with tablets alone. A large cyst with a thick wall usually needs a procedure or surgery as well.

How long is albendazole taken after surgery?

Often one to three months. Your surgeon sets the length from the operation, the report and your scans.

Why take it with fatty food?

The body absorbs albendazole far better with some fat in the meal. Milk, ghee or a normal home meal is enough.

Do I need blood tests while taking it?

Yes. Liver tests and blood counts are checked at regular visits, because the tablet can raise liver enzymes or lower blood counts.

Can I stop if I feel fine?

No. Feeling well does not mean the worms are gone. Stopping early can allow a new cyst to grow.

Is it safe in pregnancy?

No. Pregnant women should not take albendazole. A woman who plans pregnancy should tell the doctor before starting.

Can a hydatid cyst come back after surgery?

It can, which is why the tablets are continued and follow-up scans are done. A returning cyst is usually small and found early.
