In short
A liver adenoma is a growth made of liver cells. It is not cancer when it is found. It is usually linked to hormones: the contraceptive pill in women, and body-building steroids in men.
Two things make it different from other harmless liver lumps. It can bleed, and a small number turn into liver cancer over time.
The type of adenoma, its size and whether the patient is a man or a woman decide the plan. Some are watched after the hormones stop. Others are removed.
An adenoma that changes becomes a liver cancer called hepatocellular carcinoma. How that cancer is found and treated is set out on the liver cancer (hepatocellular carcinoma) treatment in Ahmedabad page.
A liver adenoma is a single growth of liver cells, usually in a liver that is otherwise healthy. Most are found by chance, on a scan done for something else.
The liver can carry several kinds of lumps. Most of them never need treatment. A simple cyst, a blood-vessel lump called a haemangioma, and a lump called FNH are the common ones. The adenoma sits apart from them. Its cells have a fault that lets one group grow on its own.
Some people feel a dull ache under the right ribs. Others notice nothing. A sudden, severe pain in the same place can mean the adenoma has bled. That needs a hospital the same day.
Two risks set the adenoma apart. It can bleed inside itself or into the abdomen, and it can change into liver cancer. Both risks can be estimated before anything is decided.
Neither risk is the same for every patient. A small adenoma in a woman, of a low-risk type, rarely does either. A large one, or any adenoma in a man, carries more. So the question is never simply “is it an adenoma?” It is “which adenoma, how large, and in whom?”
Hormones feed most adenomas. That is why the first questions at the consultation are about pills and supplements, not the scan.
| Driver | What it means |
|---|---|
| The contraceptive pill | The classic cause, in young women. The risk rises with longer use. Stopping the pill is the first step. |
| Body-building steroids | A rising cause in young men who train at the gym. These adenomas are more likely to turn into cancer. |
| Pregnancy | Pregnancy hormones can make an adenoma grow or bleed. It is planned for in advance. |
| Extra weight and diabetes | Linked to one type that tends to bleed. Losing weight is part of the treatment. |
| Glycogen storage disease | A rare inherited sugar-storage problem. It often causes several adenomas, which need closer watching. |
Men are often not asked about steroid injections or gym supplements, and rarely mention them. Every man with a liver adenoma should be asked directly, because it changes the plan.
Adenomas are now sorted into types by the faults inside their cells. The type tells the team how the adenoma is likely to behave. An MRI gives strong clues. Special stains on a biopsy can confirm it.
| Type | Risk of bleeding | Risk of turning into cancer |
|---|---|---|
| Fatty type (HNF1-alpha) | Low | Lowest of all |
| Inflammatory type | Higher; it is the commonest type and is linked to weight | Low, unless it also carries the beta-catenin fault |
| Beta-catenin type | Varies | Highest; this is the type that is removed |
| Sonic hedgehog type | High | Low |
| Unclassified | Varies | Varies |
Three things drive the risk of cancer: being a man, the beta-catenin type, and a size over 5 cm. Most of the plan follows from those three.
The lump that looks most like an adenoma is FNH. FNH needs no treatment at all. So telling the two apart is the most useful test in the whole work-up.
A biopsy carries a small risk of bleeding. It can also miss a small area of change. So it is not done when the decision to remove the lump is already clear.
The first step for almost everyone is to stop the hormone driver and treat extra weight. Many adenomas in women shrink or stop growing once the pill is stopped.
A woman with a small adenoma is usually scanned again about six months after stopping the pill. That scan shows which path she is on.
Bleeding is the event this plan tries to prevent. It shows as sudden pain under the right ribs. With a large bleed there may be faintness or collapse.
A bleeding adenoma is not rushed to an emergency liver operation. The bleeding artery is first blocked from inside, through a thin tube passed from the groin. The lump is removed later, once the patient is stable.
Blocking the artery this way is called embolisation. It stops most bleeds without opening the abdomen. An operation in a calm, planned setting is safer than one done in shock.
A woman with an adenoma who is planning a pregnancy needs a planned talk before she conceives. Pregnancy hormones can make an adenoma grow and bleed.
Some people have ten or more adenomas. This is called adenomatosis. It runs in some families and is seen with extra weight and with sugar-storage disease.
With many adenomas, the risk follows the largest and highest-risk ones, not the count. Those are removed or destroyed, and the small ones are watched.
Heat treatment through a needle, called ablation, can treat a small adenoma in a hard-to-reach place. It does not give tissue to examine. So a higher-risk adenoma in a fit patient is still removed. A liver transplant is kept for the rare patient with sugar-storage disease whose many adenomas cannot be removed.
When an adenoma is removed, only the part of the liver that holds it is taken out. The rest of the liver is kept. A rim of normal liver is taken around the lump, because a hidden area of change cannot always be ruled out first.
An adenoma that is watched is checked with MRI and the AFP blood test. The first scan comes about six months after the pill is stopped, then at one year, then once a year.
Once it has stayed steady or shrunk for five years, the gap can widen to every two years. Four findings move the plan to an operation:
| Question | Why it matters |
|---|---|
| Is this an adenoma or FNH, and how sure are we? | FNH needs nothing. An adenoma needs a plan. |
| Which type of adenoma is it? | The beta-catenin type is removed. Low-risk types in women may be watched. |
| Should I stop the pill or any supplement today? | Stopping the hormone driver is the first treatment for nearly everyone. |
| What is the plan if I want to become pregnant? | A larger adenoma is usually dealt with before the pregnancy. |
No. When it is found, a liver adenoma is not cancer. A small number change into liver cancer over time, mostly in men and in the beta-catenin type. That is why each one gets a plan.
Yes, in nearly every case. Stopping the pill is the first treatment. Many adenomas in women shrink or stop growing afterwards. Talk about another form of contraception at the consultation.
Adenomas in men turn into cancer more often than those in women, at any size. Many are linked to body-building steroids. So the advice for a man is to remove it.
Some do shrink, mostly in women once the pill is stopped and extra weight is lost. That is checked with MRI over months, not assumed.
Often yes, with a plan. A small, steady adenoma is followed with ultrasound through the pregnancy. A larger one is usually removed before conceiving.
Almost never. A transplant is kept for a rare patient with an inherited sugar-storage disease whose many adenomas cannot be removed.
