In short

No single test proves bowel TB. The diagnosis is built from the colonoscopy, the biopsy, germ tests, scans and sometimes the response to treatment.

Bowel TB and Crohn’s disease look alike but need opposite treatment. Steroids can spread hidden TB, and TB tablets delay care for Crohn’s.

A negative test never rules out TB. When the picture stays unclear, a short trial of TB tablets is judged by a repeat colonoscopy.

This post is about how the diagnosis is made. For the whole picture of bowel TB, from tests to tablets and surgery, see our intestinal tuberculosis treatment page.

Which tests diagnose bowel TB?

Doctors use a set of tests, not one. Blood tests, a chest X-ray, a scan, a colonoscopy with biopsy and germ tests each add one piece.

The aim is to see the bowel, take a tissue sample from it, and look for the germ in that sample. Each step answers a different question.

The usual tests for suspected bowel TB and what each adds
TestWhat it tells usLimit
Blood tests, including HIV and sugarAnaemia, low protein, and conditions that raise the chance of TBNot specific to the bowel
Chest X-rayLung TB at the same time, which supports the diagnosisA normal X-ray does not exclude bowel TB
CT or MR scan of the bowelThick bowel wall, swollen nodes, fluid in the belly, complicationsCan look like Crohn’s or a growth
Colonoscopy with biopsyThe ulcers themselves, and tissue for the labA thin bite may miss deeper disease
Germ tests on tissue or fluidProof of TB when positiveOften negative even in true TB
Fluid from the belly, if presentA particular fluid pattern points to TBNeeds a tap and a lab report

Please bring every earlier scan and report. A good work-up avoids repeating tests that were already done well.

Why is bowel TB hard to prove?

The germ is scarce in the bowel wall, and a biopsy takes only the inner lining. So a negative test is common even when TB is present.

In the lungs, a person coughs up thousands of germs. In the bowel, far fewer germs sit in a patchy, deep layer of the wall. A forceps bite from the surface can miss them. This is why doctors say the tests are strong when positive but weak when negative.

  • A positive germ test or a typical cheesy-looking (caseating) granuloma on the slide confirms TB
  • A negative result does not exclude TB, and it is the most common mistake to treat it as proof of absence
  • Many biopsies from the ulcer edge and the ileum raise the chance of finding something
  • Tissue is split so that the same sample goes for the microscope and for germ tests

The germ tests include a smear, a rapid test called GeneXpert, a culture and sometimes a gene test. GeneXpert also shows whether the germ resists rifampicin, a key TB tablet. A positive result is decisive. A culture is slower, taking weeks, but gives the full drug-sensitivity picture. Blood tests for past exposure to TB are of little use in India, because many healthy people are positive.

What does the colonoscopy show?

The doctor looks at the shape of the ulcers and the valve at the end of the small bowel. These give the first clue to TB or Crohn’s.

TB tends to make ulcers that run across the bowel, like rings. The valve between the small and large bowel often looks wide open and fixed. In Crohn’s, ulcers usually run along the length of the bowel, and the lining may look like cobblestones. Disease is also patchy in Crohn’s, with healthy gaps between sick stretches.

No one sign decides the matter. It is the sum that counts. The next table puts the main clues side by side.

How are TB and Crohn's told apart?

Doctors weigh clues from the history, the scope, the scan and the slide. The more clues point one way, the surer the answer.

Clues that lean towards bowel TB or towards Crohn’s
Where we lookLeans towards TBLeans towards Crohn’s
Ulcer shapeAcross the bowel, ring-likeAlong the bowel, or small sores
Valve at the bowel junctionWide open and fixedNarrow and stiff
Spread of diseaseA few segments near the junctionPatches with healthy gaps
Lining of the bowelNodular, with polyp-like lumpsCobblestone look
Around the anusUncommonFissures and fistulas are common
CT scanShort thick segment, dead-looking nodes, fluidLong thick segment, a comb-like pattern in the fat
Under the microscopeLarge joined granulomas with cheesy centresSmall, scattered granulomas without a cheesy centre
Fever and night sweatsMore likelyLess likely

Some doctors turn these clues into a score. In India TB is common. So the safe habit is to rule out TB before any strong Crohn’s drug is used.

What if the tests stay unclear?

If the tests stay unclear, a fixed trial of TB tablets is sometimes given. The trial is judged by a repeat colonoscopy at two to three months.

Feeling better is not proof. Symptoms can ease in both diseases for a while. What counts is whether the ulcers have healed when the doctor looks again.

  • Ulcers healed means TB is very likely, and the full course of tablets is completed
  • Ulcers unchanged means the diagnosis is rethought, and Crohn’s disease is looked at again
  • Biopsies are repeated if the picture still does not fit

A keyhole look at the belly, with a small biopsy of its lining, is the most direct route to tissue. It is kept for stubborn cases and for those with belly fluid. Once TB is accepted, our post on how long bowel TB medicines last explains the course.

Why must the diagnosis be right first?

The two treatments work against each other. A wrong call in either direction can do real harm, which is why the work-up is careful.

What goes wrong when the diagnosis is wrong
MistakeWhat happensHow it is avoided
TB treated as Crohn’s with steroids or biologic drugsTB can spread through the body and the bowel can burstRule out TB before any such drug
Crohn’s treated as TBThe disease keeps growing and tablets add side effectsCheck ulcer healing at two to three months
TB lump operated on as a growthA big operation that tablets alone could have avoidedTake tissue before any big operation
Drug-resistant TB missedStandard tablets fail and the germ keeps spreadingAct on the rifampicin result of GeneXpert
One shallow biopsy trustedA false sense that there is no TBMany deep biopsies, with germ tests on the same tissue

This is why a surgeon who sees both diseases reads the reports together. Our posts on bowel TB symptoms and warning signs and on complications and follow-up cover what comes before and after the diagnosis.

What should you bring and ask?

Bring your scans, colonoscopy report, biopsy slides and blocks, and a list of every drug you have taken. Ask what is proven and what is only likely.

It helps to ask these questions at your visit:

  1. Was the tissue sent for the microscope and for germ tests?
  2. Was GeneXpert done, and did it check for rifampicin resistance?
  3. Has TB been ruled out before any steroid or biologic drug?
  4. If tablets are tried, when will the scope be repeated?
  5. Was a tissue sample taken before an operation was advised for a bowel lump?

Dr Harsh Shah works with your physician and TB doctor. They guide the tablets and the Crohn’s drugs. We are there when the bowel needs a surgeon, or when a second opinion will help.

Frequently asked questions

Can one test prove intestinal TB?

No. Every test misses some cases. So the diagnosis is built from the colonoscopy, biopsy, germ tests, scans and the response to treatment, read together.

Does a negative biopsy rule out bowel TB?

No. The germ is scarce in the bowel wall and the biopsy takes only a thin shallow bite. A negative result does not exclude TB, and the doctor looks at the whole picture.

What is GeneXpert and why is it done?

It is a rapid test that finds TB germ material in tissue or fluid and shows whether the germ resists rifampicin. A positive result is strong proof. A negative result does not exclude TB.

Why does it matter whether it is TB or Crohn's?

The treatments are opposite. Steroids or biologic drugs can make hidden TB spread, and TB tablets given for Crohn's delay the right care.

Is a blood test for TB enough?

No. In India many healthy people test positive because of past exposure. These blood tests cannot tell old exposure from active bowel disease.

What is a trial of TB tablets?

When the tests stay unclear, the doctor may give TB tablets for a fixed period. The result is judged by a repeat colonoscopy at two to three months, not by how you feel.

Is surgery needed to diagnose bowel TB?

Rarely. Most cases are settled with colonoscopy, biopsy and scans. A keyhole look at the belly with a biopsy is kept for cases that stay unclear.

Can a bowel lump be TB and not cancer?

Yes. TB can look like a growth on a scan. A tissue sample must be taken before any big operation is planned.
