In short
Rectal cancer most often shows as blood in the stool. Next come a change in bowel habit and a feeling of incomplete emptying.
Bleeding is wrongly called piles for months, mostly in people under fifty. Bleeding that lasts six weeks needs a finger examination and a scope.
Early rectal cancer often causes little trouble. Pain, weight loss and a blocked bowel usually come late, so waiting for them costs time.
If you already know the diagnosis, the plan of care is on the middle rectal cancer surgery page. This post is for the earlier stage, when you only know that something is wrong.
The usual first signs are blood, a change in how you pass stool, and a false urge to go. None of them is specific, and that is the trap.
| Sign | How people describe it | What to know |
|---|---|---|
| Blood with stool | Red blood on the stool, mixed in, or on the paper | The most common first sign. Often blamed on piles |
| Change in bowel habit | Looser stool, more trips, sudden urgency, or both ways in turn | New and lasting for weeks, not one bad day |
| False urge (tenesmus) | Feeling you must go, but little or nothing comes | More typical of a rectal growth than of piles |
| Incomplete emptying | A feeling that stool is stuck or a lump is inside | The growth itself can feel like stool |
| Thin or pencil-like stool | Stool that comes out narrow | Suggests the passage is narrowing |
| Mucus from the back passage | Slime on the stool or alone | Seen with some growths, and with other bowel problems |
Any one of these can have a harmless cause. A new one that stays for weeks is what matters.
Piles and rectal cancer both bleed. Piles are common, so they get the blame. The delay that follows is the real danger.
Audits from Indian centres describe months of delay in younger patients. The cause is rarely the cancer. It is the assumption. Piles and cancer can also exist together, so a pile found on examination does not clear the rectum above it.
Pain, weight loss, a blocked bowel and heavy bleeding are late signs. They mean the growth has been there for some time.
A blocked bowel is an emergency. Go to the nearest hospital first, then call us.
Seek an examination if bleeding lasts about six weeks, whatever your age. Do not wait for a second episode.
Young age does not protect you. A family history is often missing, so its absence is no comfort.
The visit is short. It starts with questions and a gentle finger examination, then usually a scope with a small tissue sample.
What comes after a diagnosis depends on where the growth sits. For a low growth, see lower rectal cancer: sphincter or stoma.
Before any treatment, we note how your bowel, bladder and sexual function work today. These notes guide the operation and the talk about consent.
These answers help decide if the muscle of the back passage can be kept. They also guide the first step: radiation, chemotherapy or surgery.
Yes, sometimes. A stool blood test, a colonoscopy for family history, or a polyp found on a routine scope can catch it early.
Cancers found without symptoms are usually earlier and easier to treat. In India there is no national screening, so most are found because of symptoms. That is one more reason not to ignore them.
No. Piles are the common cause, but rectal cancer bleeds in the same way. Bleeding that lasts weeks needs an examination and a scope.
Yes. It is seen under fifty, and the delay is often longer because it is called piles. Age should not stop a scope.
No. Early rectal cancer often does not hurt. Pain usually means the growth has been there longer.
It is a feeling that you must go, with little or nothing coming. It is more typical of a rectal growth than of piles.
If bleeding lasts about six weeks, get examined. Do not wait for it to repeat. Heavy bleeding or a blocked bowel needs care at once.
If a growth is seen, the rest of the bowel is still checked. If the short scope is normal, a full colonoscopy looks higher up.
