In short
Intestinal TB can narrow the bowel. Many narrowings improve with a full course of TB medicines, but a scarred narrowing does not melt away.
An operation is kept for a bowel that stays blocked, a blockage that is complete, a hole in the bowel, or a lump that cannot be told from cancer.
The aim of surgery is to open the passage and save as much bowel as possible. The TB medicines still have to be finished.
The full plan is on the intestinal tuberculosis treatment in Ahmedabad page. This post explains narrowings and blockage in particular. It covers the causes, the tests, and when medicines, a balloon or an operation fits.
TB heals by scarring. The scar can pull the bowel wall in and leave a tight ring. This is why a person can finish the medicines and still have a blocked bowel.
A scarred narrowing is a mechanical problem, not a new infection. Giving more months of medicine does not open it. The cause of such scarring is covered on the intestinal TB treatment page.
A partly blocked bowel gives colicky pain that comes in waves, mostly after food. It often settles for a while and then returns.
People often cut down on food to avoid the pain. This leads to weight loss and weakness, which makes any operation riskier. So it is better to be seen early than late.
Tests answer three questions: is the bowel narrowed, is the cause TB, and is the narrowing active swelling or old scar.
| Test | What it shows |
|---|---|
| CT scan of the belly, with the bowel filled | Where and how long the narrowing is, thick or enlarged glands, fluid |
| Small bowel contrast X-ray | The length and number of narrowings |
| Colonoscopy with biopsies | Ulcers, the ileocaecal valve and tissue for TB tests |
| TB tests on the biopsy | Typical tissue changes, the TB germ, and drug resistance |
| Chest X-ray and blood tests | TB in the lungs, anaemia, protein and vitamin levels |
A swallowed camera capsule is avoided when a narrowing is suspected, because it can get stuck. TB and Crohn disease can look alike on scans and scope, but their treatment is opposite. So tissue is taken before any steroid or similar drug is started. Read more on Crohn disease treatment in Ahmedabad.
If the bowel is only partly blocked and the narrowing looks active, a full course of TB medicines comes first. Most people improve over weeks.
The medicines treat the infection but cannot undo scar. If symptoms of blockage keep returning after the infection is treated, the narrowing is probably scar. At that point the question is no longer medicine. It is whether the passage needs to be opened.
A balloon can stretch a short narrowing within reach of the scope. Long, many or deep narrowings in the small bowel usually need an operation.
If a lump may be cancer, tissue is needed first when possible. When it is not possible, removing the lump is safer than waiting.
Stricturoplasty widens a narrow spot and keeps the bowel. Resection removes the diseased piece and joins the healthy ends. The surgeon picks by the type of narrowing.
| Operation | What is done | Usually chosen when |
|---|---|---|
| Stricturoplasty | The narrow wall is opened along its length and stitched across, so the passage is wider | There are several short narrowings in the small bowel, and bowel length must be saved |
| Resection and join | The diseased piece is removed and the two healthy ends are stitched together | One long or tight segment, a mass at the lower end of the small bowel, a hole, or doubt about cancer |
Some people need both in one operation. Keyhole surgery may be possible in selected cases, but that is decided after the scans. Risks include a leak at the join, infection, and a narrowing that returns. Taking out a long length of small bowel can leave poor absorption. This is why saving bowel is a priority. The TB medicines are continued or completed after surgery.
If nothing passes and vomiting does not stop, go to a hospital with a surgeon at once. Do not wait to see if it settles.
In an emergency the first steps are a drip, a tube through the nose to empty the stomach, and scans. Some people settle with this and some need an operation the same day. Delay raises the risk of a hole in the bowel.
Yes. TB heals by scarring, and the scar can narrow the bowel. The lower end of the small bowel is the usual place.
If the narrowing is active swelling, often yes. If it is old scar, medicines treat the infection but cannot remove the scar.
No. Many people improve on medicines. Surgery is kept for a bowel that stays blocked, a complete blockage, a hole, or a lump that may be cancer.
The narrow wall is opened along its length and stitched across, so the passage is wider and no bowel is removed. It suits several short narrowings.
For a short narrowing within reach of the scope, a balloon can stretch it. Long or many narrowings in the small bowel usually need an operation.
When vomiting does not stop, the belly swells hard, and no gas or stool passes. Severe constant pain with fever is also an emergency.
