In short
Most stomach cancer that returns has spread through the body, so the first treatment is usually medicine. A second operation helps only in a small, carefully chosen group.
Before any plan, scans and a biopsy confirm the return. The tumour is then tested again, because its features can change.
Even when the cancer cannot be removed, bleeding, blockage, pain and weight loss can be treated. Comfort and strength are part of the plan from the start.
How stomach cancer is first tested, staged and treated is set out on the stomach cancer treatment in Ahmedabad page. This post is about what comes next when it returns.
A return, or recurrence, means cancer cells that survived treatment have started to grow again, in the same place or somewhere else.
It can happen after surgery alone, or after surgery with chemotherapy. It does not mean the first treatment was wrong. Some cells are too small to see on any scan at the time.
Most returns appear in the first two to three years, which is why the follow-up visits are closest together then. Late returns, after five years or more, are less common and often behave differently.
The pattern depends on the cell type. The diffuse or signet-ring type tends to spread over the lining of the belly. The intestinal type tends to travel to the liver.
| Site | Often seen with | What it may cause |
|---|---|---|
| Lining of the belly (peritoneum) | Diffuse or signet-ring type | Bloating, fluid in the belly, blocked bowel |
| Lymph glands far from the stomach | Either type | Back pain, a lump in the neck, swelling |
| Liver | Intestinal type | Tiredness, discomfort on the right, yellow eyes |
| Lung or bone | Either type | Cough, breathlessness, bone pain |
| The join or the stomach bed | More often when the first margin was close | Food sticking, vomiting, bleeding, pain |
Many returns have no symptom at first and are found on a routine scan. That is a reason to keep the follow-up dates, set out on the post about follow-up after stomach cancer treatment.
A suspicious scan or a rising symptom is checked in steps. The aim is to prove the return and to learn everything about the new tumour before treating it.
The biopsy is then tested again for HER2, MSI or mismatch repair, and PD-L1. These can differ from the first tumour, and they decide which medicine will work. A wider gene panel is added when it may open a treatment.
Surgery for a return helps only when the disease sits in one spot that can be removed completely. The person must also be fit enough for it.
| Situation | What is usually needed |
|---|---|
| A few spots in the liver | A small number, in one part of the liver, that respond to medicine; removal or burning by probe |
| One or a few lung nodules | No other disease on PET-CT; a keyhole wedge removal or lobectomy |
| Far glands, such as behind the main artery | Few glands, good response to medicine, fit patient |
| Return at the join | Disease confined to the join; completion gastrectomy if it is reachable |
Each of these is a major decision, and the benefit is smaller than for the first operation. A tumour board discussion, with the surgeon, medical oncologist and radiologist, is how the choice is made.
Sometimes cancer is found already spread, and medicine shrinks it well. If everything visible then becomes removable, an operation after the medicine is called conversion surgery.
Conversion surgery is for people whose cancer responded very well and who stay fit. It is not for everyone with spread, and a trial in unselected patients did not show a gain.
The treatment of spread cancer in general is covered on the post about treatment of stomach cancer that has spread.
Cancer on the peritoneum is a frequent form of return, and a hard one to treat. A few carefully chosen people may be offered a heated chemotherapy operation.
For limited disease, a large operation called CRS and HIPEC may be offered. It needs a low spread score and a good response to medicine. The surgeon removes what is visible and bathes the belly in warmed drug. It carries a real risk and needs a specialised centre and a strong recovery team.
For disease that cannot be removed, a keyhole procedure called PIPAC puts a mist of drug into the belly. It is repeated every few weeks, mainly to control symptoms, and it is available at only a few centres in India.
For most people, a return is treated with medicine first. Which medicine depends on what was used before, how long ago, and the tests on the new tumour.
Response is checked on scans every two to three months. If a course is not working or the side effects outweigh the gain, the plan changes. It is always fine to ask why a medicine is being chosen.
A cancer in the stomach remnant, more than five years after a partial gastrectomy, is treated as a new stomach cancer. It is not seen as a return.
A stump cancer is treated with the aim of removing it for good. The remnant is taken out with its glands, and the bowel is joined by a Roux-en-Y loop.
Scans first check that it has not spread. Medicine before or after the operation is chosen as for a first stomach cancer. This is set out on the post about treatment of locally advanced stomach cancer.
When the cancer cannot be removed, treating symptoms is not giving up. It lets people eat, sleep and stay at home for longer.
| Problem | What can be done |
|---|---|
| Bleeding | Clipping at endoscopy, a blocking procedure by the radiologist, or short radiotherapy |
| Blocked food passage | A stent through the narrowing, or a bypass operation in selected people |
| Pain | Medicines stepped up carefully, a nerve block, or radiotherapy to the painful spot |
| Fluid in the belly | Draining it when needed, or a small drain kept in place |
| Anaemia | Iron by tablet or drip, or blood when needed |
| Weight loss and weakness | Dietitian advice, nutrition drinks, or feeding by a tube when eating is not possible |
A palliative care team works alongside the cancer treatment, not only at the end. Starting early improves comfort, and often strength too.
Do not wait for the next visit if any of these appear:
Yes. Medicine is the main treatment for most returns, and a few people can have an operation as well. Even when it cannot be removed, symptoms can be treated and strength protected.
Most returns appear in the first two to three years, which is why follow-up is closest then. A return after five years is less common.
Usually yes. A new biopsy proves the return and lets the tumour be tested again for HER2, MSI and PD-L1, which can change and guide the medicine.
Only when the return is limited to one removable spot, such as a few liver spots, a lung nodule or the join, and you are fit. Many people are treated with medicine alone.
It is an operation after medicine has shrunk cancer that had spread, when everything visible has become removable. It is for people who responded very well, not for everyone.
Call if you vomit, pass black stools, cannot eat, have yellow eyes, a swelling belly, new pain or a cough that does not settle.
