In short

An endoscopic procedure on the pylorus is a later step. It is considered only after food changes and a fair medicine trial have not worked.

The idea is simple. In some people the stomach outlet stays tight and food cannot leave. Cutting the muscle of that outlet lets food pass.

Choosing the right person decides the result. A person with a tight outlet may gain a lot. A person without it may gain nothing.

All the treatment options, including surgery on the pylorus, are on the gastroparesis treatment in Ahmedabad page. This post explains the procedures done through the mouth.

Who should think about a procedure

A procedure comes after the basics. Food changes and a fair medicine trial must come first, and any slowing drug must be cleared.

  • Sugar is brought under control, and opioid painkillers are reduced.
  • GLP-1 injections are stopped or cut down, if you take them. Their slowing effect can fade.
  • Diet and medicines are tried at the right dose for long enough. These steps are in the post on diet and medicines for a slow stomach.
  • Delayed emptying is proven on a proper test. How this is done is in the post on how gastroparesis is diagnosed.
  • A blockage is ruled out by endoscopy or scans.

Treating a drug effect with a cut that cannot be undone would be a clear mistake. That is why these steps are never skipped.

The idea behind pyloric treatment

The pylorus is the valve at the stomach exit. In some people it stays tight, and food waits behind it.

  • A tight valve works like a partial blockage, even though nothing is blocking it.
  • Opening the valve lets the stomach empty more freely.
  • People whose valve is normal do not gain from this. The treatment is for those with a tight outlet.
  • That is why choosing the right person matters more than the procedure itself.

Botulinum toxin injection: a test, not a treatment

Botulinum toxin relaxes the pylorus for a few months. Careful trials found that it does not ease symptoms better than a dummy injection.

  • It was once used widely, based on early reports without a comparison group.
  • Two controlled trials then showed no real gain in symptoms.
  • Guidelines advise against it as a treatment.
  • Some centres still use it as a short trial of the idea. The result is not reliable, and a poor response does not rule out later benefit.

It is still given often in India. If it is offered to you as treatment, ask why.

Finding the right person

No single symptom shows a tight pylorus. A measurement of how well the valve stretches comes closest.

Tests used to choose a pyloric procedure
TestWhat it tells youLimit
Stretch measurement of the pylorus (EndoFLIP)How stiff the valve isNot available in every centre
Gastric emptying scanConfirms the stomach is slowDoes not show where the problem lies
Pressure study of the stomach and bowelHow the muscle is workingInvasive; used in few centres
Response to a temporary openingWhether symptoms easeNot fully proven; the effect is brief

A stiff valve on the stretch test points to a better result. Where the test is not available, the doctor relies on the full picture. This includes retained food and the emptying scan.

G-POEM: how the procedure is done

G-POEM stands for gastric per-oral endoscopic myotomy. The pyloric muscle is cut from inside the stomach, with no cut on the skin.

  • It is done under general anaesthesia.
  • A thin camera goes through the mouth into the stomach.
  • A small tunnel is made under the lining, near the exit.
  • The tight muscle of the pylorus is divided inside the tunnel.
  • The entry point is then closed with clips or stitches.

Most people stay in hospital for a day or two. Drinks start first, then soft food. A trained team that does this often matters a great deal.

What results to expect

A trial with a dummy procedure showed that G-POEM helps more than placebo in severe cases. The benefit is real, but it can fade over time.

  • Many people feel clearly better within weeks.
  • For some, the gain slowly weakens over the first year.
  • Some have no benefit at all, mostly when the valve was not the problem.
  • People with diabetes or with earlier stomach surgery often respond better than those with no known cause.
  • Pain-led symptoms with a normal valve respond poorly.

No operation can promise a result. An honest talk about these limits comes before consent.

Risks of G-POEM

G-POEM is safe in experienced hands. The most common disappointment is no benefit, not a serious complication.

  • Bleeding inside the tunnel can happen. It is usually controlled during the procedure.
  • A leak or hole at the entry point is uncommon. It is treated with clips, antibiotics and, rarely, an operation.
  • An ulcer may form near the pylorus. Acid-lowering tablets treat it.
  • Gas can collect in the belly. It is usually harmless and is let out if tight.
  • A collection of infection is rare. Fever or sharp pain after the procedure needs a scan.
  • Some people feel dumping, meaning very fast emptying, with weakness after meals. Diet changes usually help.

G-POEM or surgery on the pylorus?

Both open the valve. The choice depends on whether anything else must be done at the same time.

  • G-POEM suits a person whose main problem is the tight valve. There is no cut on the belly.
  • Keyhole surgery on the pylorus suits a person who also needs a feeding tube into the bowel or a stomach stimulator.
  • Both are permanent. Neither can be undone.
  • A repeat G-POEM, or surgery, is sometimes possible if the benefit fades.

These operations are set out on the gastroparesis treatment page.

When it does not work

If G-POEM does not help, repeating it is not the first move. The cause is looked for again.

  • Was delayed emptying really proven, and were look-alike conditions ruled out?
  • Was a slowing drug missed, such as an opioid or a GLP-1 injection?
  • Was the valve truly tight on a stretch test?
  • Was the muscle cut fully? A repeat look inside can show this.
  • What is the main symptom now? Nausea may suit a stomach stimulator. Poor intake may need tube feeding. Pain may need nerve-calming medicine.

Follow-up is covered in the post on gastroparesis complications and follow-up.

Questions to ask at your visit

  • Have we cleared every slowing drug and tried the medicines fairly?
  • Is my emptying test proven, and is my valve tight?
  • Is G-POEM or surgery better for me, and why?
  • What are the chances that it will not help me?
  • What happens next if the benefit fades?

Frequently asked questions

When is a procedure considered for a slow stomach?

After food changes and a fair medicine trial have not worked, and slowing drugs are cleared.

What is G-POEM?

The pyloric muscle is cut from inside the stomach through the mouth. There is no cut on the skin.

Is botulinum toxin a good treatment for a slow stomach?

No. Trials show no real gain in symptoms. It is at most a short test.

How do doctors choose who gets G-POEM?

A stretch test of the pylorus, an emptying scan and the full picture are used together.

Does G-POEM work for everyone?

No. Some people gain a lot, some gain for a while, and some gain nothing.

What if G-POEM does not help?

The cause is looked at again. A repeat G-POEM, surgery or another plan may follow.
