In short

The small bowel lies beyond the reach of a gastroscopy and a colonoscopy, so finding a tumour needs a planned order of tests, not one test.

A scan of the bowel usually comes first. A capsule camera is used only after we know the passage is open, and a scope is used to take tissue.

Tissue decides the type of tumour. In India we also test for gut tuberculosis at the same time, so that the wrong operation is avoided.

For how these tumours are treated once found, read our small bowel cancer treatment page. This post explains the tests, and why their order matters.

Which test should come first?

The first test depends on the question. A suspected lump needs a scan of the bowel. A hidden bleed with normal scopes may need a capsule. Tissue needs a scope.

Match the test to the question
The questionFirst testWhy
Is there a lump or narrowing?CT or MR enterographyShows the wall, glands, liver and belly in one scan
Is there a hidden bleeding spot?Capsule camera, after the passage is checkedSees small lesions on the inner lining
I need tissue or a mark on the lesionDeep scope (balloon enteroscopy)Takes a biopsy and can mark the spot with ink
What type is it, and how far has it spread?Bloods, biopsy and special stainsType decides treatment

The most common mistake is the wrong order: a capsule sent into a narrowing, or a third colonoscopy when the small bowel was never imaged.

What does a scan of the small bowel show?

CT or MR enterography fills the bowel with fluid and shows the wall, the fat around it, the glands and distant spread in a single visit.

  • CT enterography is quick and widely available, and is our usual first scan.
  • MR enterography gives finer detail of the bowel fat and avoids radiation, so it suits younger patients and repeat scans.
  • In an emergency, a plain CT of the belly shows where the blockage is and why.
  • Enteroclysis, where fluid is passed through a thin tube, is kept for scans that remain unclear.

The shape on the scan gives clues before the biopsy returns.

What the scan picture may suggest
Scan pictureMay suggest
Short ring-like narrowing with sharp edgesAdenocarcinoma
Mass in the bowel fat with streaks pulling the bowelNeuroendocrine tumour
Thick wall that widens the bowel instead of narrowing itLymphoma
Round mass growing outward with little blockageGIST
Bowel folded inside bowelA tumour acting as the lead point

These are patterns, not proof. Tissue gives the final answer. See our page on GIST treatment for what follows a GIST result.

When is a capsule camera safe?

A capsule camera is safe only when the passage is known to be open. It sees the lining well but cannot take tissue or treat.

  • With blockage symptoms or a known narrowing, we scan first, or use a dissolving test capsule.
  • A camera stuck above a tumour can turn an elective test into an emergency operation.
  • A capsule helps most for hidden bleeding, once both end scopes are normal.
  • If it shows a lesion, the next step is a deep scope to take tissue.

For a brisk bleed, a CT scan with contrast in the vein is faster than waiting for a capsule to travel through the gut.

How is tissue taken from the small bowel?

Tissue comes from a scope when the lesion is reachable, a needle when a spot elsewhere is easy to reach, or from the operation itself.

  • A gastroscope reaches the first parts of the duodenum. A side-viewing scope is needed to see the opening of the bile duct.
  • A colonoscope can enter the last part of the ileum and take samples.
  • A deep balloon scope reaches further, takes a biopsy and can tattoo a small lesion so the surgeon finds it later.
  • For a duodenal or ampullary growth, an ultrasound scope shows how deep it goes.

A shallow pinch biopsy can miss a growth under the lining. We take deeper samples when needed, and tell the pathologist what we suspect. Read about tumours of the duodenum and ampulla to see what a scope can reach.

Which blood tests and special tests help?

No blood test proves a small bowel cancer. They point to the type, and the tissue stains choose the treatment.

Tests that guide the type of tumour
Suspected typeTests added
AdenocarcinomaIron tests, tissue stains for repair-gene faults (MSI), and HER2
Neuroendocrine tumourChromogranin A, 24-hour urine 5-HIAA, a special PET scan, and a heart check before surgery
LymphomaLDH, tissue large enough for full typing, and a PET scan
GISTTissue gene test for KIT and PDGFRA

Some results mislead. Acid-reducing tablets and kidney weakness can raise chromogranin A. Certain foods, such as banana and pineapple, raise the urine test. We plan the collection so the result is clean.

Why is tuberculosis tested at the same time?

In India, gut tuberculosis can look just like a tumour on a scan. We test for both together before choosing an operation.

  • Tissue goes for tuberculosis stains, culture and a rapid gene test (GeneXpert) along with the cancer slides.
  • A long smooth narrowing with calcified glands leans towards tuberculosis.
  • A short, irregular narrowing with solid glands and weight loss leans towards cancer.
  • Tablets for tuberculosis are never allowed to delay an operation that the picture clearly needs.

This step prevents a large cancer operation for tuberculosis, and a late operation for cancer treated as tuberculosis.

What if it first shows up as an emergency?

If a tumour first appears as a blockage, tear or bleed, one CT scan guides the operation, and full staging follows after recovery.

  • A first blockage with no earlier belly operation, or any blockage with a mass, is treated as a cancer operation.
  • Folded bowel in an adult is removed in one piece, not pushed back.
  • Tissue goes for cancer and tuberculosis tests together.
  • Scans to stage the disease are completed once you are stable.

Earlier signs that bring people in are described in our post on small bowel cancer symptoms.

What should you bring to the visit?

Bring every earlier scan, scope and blood report, and tell us about past tuberculosis, earlier operations and family cancers.

  • Blood counts and iron tests from the last year.
  • Discs or reports of every CT, MRI and ultrasound.
  • Reports of any gastroscopy, colonoscopy or capsule test.
  • A list of your medicines, including acid-reducing tablets.

Frequently asked questions

Why is a scan done before a capsule camera?

A capsule can get stuck above a narrowing and turn a test into an emergency operation. A scan of the bowel shows any narrowing first. Only then do we use the capsule.

What is a CT or MR enterography?

You drink a special fluid that fills the bowel, and then a scan is taken with contrast in the vein. It shows the bowel wall, the glands around it, the liver and the rest of the belly in one go.

Is a normal gastroscopy and colonoscopy enough to rule out a small bowel tumour?

No. These two scopes reach only the two ends of the gut. If your blood count is low in iron and both are normal, the next step is imaging of the small bowel, not a third scope of the same places.

Why is a biopsy needed if the scan already shows a tumour?

A scan shows a lump, not what it is. The cell type decides the treatment, so tissue is needed. In India we also test the tissue for tuberculosis at the same time.

What is MSI testing on a small bowel cancer?

It is a test on the tissue that looks for a repair fault in the cancer cells. It helps decide whether newer drug treatment can work, and it can point to a family-level gene condition.

Can tuberculosis of the gut look like cancer on a scan?

Yes. A long narrowing, swollen glands and a thick bowel wall can look the same. Tissue tests for tuberculosis are done together with tests for cancer so that the wrong operation is avoided.

Which blood tests help in a suspected small bowel tumour?

A blood count with iron tests is done in everyone. If a hormone-type tumour is suspected, we add chromogranin A and a 24-hour urine test. For lymphoma, LDH is checked. No blood test alone proves a cancer.
