In short
Treatment of a slow stomach is a sequence. First fix the causes you can fix. Then change the food. Then add a medicine.
Small, soft, low-fat meals help most. Liquids pass more easily than solids, so they can fill the gap when solid food will not stay down.
Each medicine has a limit. Metoclopramide is for short courses. Domperidone and erythromycin need a heart tracing. Your doctor sets the dose and the time.
All the treatment options, including the pyloric procedures, are set out on the gastroparesis treatment in Ahmedabad page. This post covers the first steps: food and medicines.
Before any new tablet, look for what is slowing the stomach. Some causes can be removed. This step is missed often.
Many people called “resistant” have not yet had this step done. How the problem is confirmed is explained on the post about how gastroparesis is diagnosed.
Food is the first treatment and the safest one. Small, soft, low-fat meals ask less of a weak stomach.
| Rule | Why it helps |
|---|---|
| Four to six small meals a day | Less food for the stomach to clear at one time |
| Low fat, especially solid fat such as ghee and fried food | Fat slows the stomach |
| Low in tough fibre, such as raw salad, skins and seeds | Less leftover residue, and a lower chance of food balls |
| Soft, mashed or blended food | Small particles leave the stomach more easily |
| Sit upright and walk a little for one to two hours after a meal | Gravity helps emptying |
| Enough fluids, and a vitamin check | Poor intake lowers iron, B12 and vitamin D |
A trial in people with diabetes showed that a small-particle diet eased nausea, vomiting, fullness and bloating. A dietitian helps you keep it up and keeps nutrition adequate.
Liquids leave the stomach by a different route that is often still working. More of your calories can come as liquid or semi-solid food.
These medicines are called prokinetics. They make the stomach empty faster. The relief of symptoms is smaller and less certain than the change in emptying.
| Medicine | How it works | Main caution |
|---|---|---|
| Metoclopramide | Speeds emptying and calms nausea | Movement side effects; short courses only |
| Domperidone | Speeds emptying; does not act on the brain | Heart rhythm; ECG needed |
| Erythromycin | Strong trigger for stomach contractions | Effect fades; heart rhythm; many drug interactions |
| Prucalopride | Acts on gut nerve receptors | Used off-label for this problem |
| Levosulpiride, itopride | Widely used in India | Fewer good trials; levosulpiride has movement and hormone effects |
A medicine is taken before meals, usually in liquid form where possible. If a tablet is vomited, a liquid or another route may work better.
Metoclopramide is the main approved medicine for this problem. It helps nausea and emptying. It can also cause movement problems that may not go away.
Domperidone does not reach the brain in a meaningful way, so it spares you the movement problems. Its risk is to the heart rhythm.
Erythromycin is the strongest short-term prokinetic. It suits a hospital stay during a flare. Over weeks, the effect fades.
Some people have mostly nausea, with near-normal emptying. In them, medicines for nausea carry the load.
A procedure is considered only after food changes and a proper medicine trial have failed. A proper trial means the right dose, for long enough.
Fix the causes you can, such as high sugar, opioids and GLP-1 injections. Then change the food. Medicines come after that.
Small, soft, low-fat meals eaten often. Blended food and liquids pass more easily than raw salad or fried food.
Only for short courses, often about three months at most. Your doctor sets the limit.
It can change the heart rhythm. An ECG is done before and during treatment.
They calm symptoms but do not speed the stomach. They help when nausea is the main trouble.
After food changes and a proper medicine trial have failed. A proper trial is the right dose for long enough.
