In short
Gastroparesis means the stomach empties too slowly, with no blockage. Three things must be found together. These are the symptoms, a test that shows slow emptying, and proof that nothing blocks the outlet.
The order matters. A scope test to rule out a blockage comes first. The emptying scan is done only after that, and after medicines and blood sugar have been put right.
Other conditions look the same and need different care, so the label should not be given on symptoms alone.
Treatment, including the pyloric procedures for selected people, is set out on the gastroparesis treatment in Ahmedabad page. This post explains how the diagnosis is made.
The usual picture is nausea, early fullness and a heavy stomach after meals. Bloating is common. Some people vomit food eaten hours before.
No single symptom proves the diagnosis. The pattern, plus a test that shows slow emptying, does. Doctors may also use a short symptom score so that progress can be followed over time.
A diagnosis of gastroparesis needs all three: the right symptoms, slow emptying shown on a test, and no blockage.
| Requirement | How it is checked |
|---|---|
| The typical symptoms | History, and a symptom score |
| Slow emptying | A gastric emptying scan, or a breath test or capsule test |
| No blockage at the outlet | Upper endoscopy, with a scan or X-ray if needed |
If the symptoms are there but the emptying is normal, the likely name is functional dyspepsia, not gastroparesis. The two overlap and one can slowly turn into the other.
Before any motility label, the outlet of the stomach must be shown to be open. This is the step a surgeon never skips.
A blockage can often be fixed. That is why it must be found first, before any long course of medicine is started.
The standard test is a scan that follows a small meal through the stomach for four hours. A shorter scan can miss the problem.
Because emptying varies from day to day, one borderline result is not final. It can be repeated before a major decision is made.
Where a scan is hard to get, a breath test or a swallowed capsule can show the same delay.
| Test | How it works | Limits |
|---|---|---|
| Four-hour scan | Tracks a labelled meal with a camera | Needs a nuclear medicine unit and careful preparation |
| Breath test | Measures a marker in the breath after the stomach empties | Less reliable with lung, liver or pancreas disease |
| Wireless capsule | A capsule records pressure and acidity as it passes | Not for people with narrowings or swallowing problems |
| Pyloric pressure study | Measures how well the outlet valve opens | Used to plan a procedure, at specialist centres only |
These tests show that emptying is slow. They do not show the cause, and they do not decide the treatment alone. That depends on the whole picture.
Several problems copy gastroparesis. Each needs a different kind of treatment, so they are ruled out one by one.
Two groups need extra care: people who have had stomach or food pipe surgery, and people with long-standing diabetes.
Some people improve over months, especially after a viral illness. A repeat test is worthwhile before an irreversible step is taken.
Simple blood tests find reversible causes and show what the vomiting has done to the body.
A complete set of papers lets the first visit move straight to a plan.
An endoscopy to rule out a blockage, then a four-hour gastric emptying scan. A breath test or capsule test can be used where the scan is not available.
A two-hour scan can miss slow emptying. The four-hour picture is the most reliable one.
Yes. High sugar slows the stomach on its own. The scan should be done with sugar well controlled, and repeated if it was not.
Medicines that speed the stomach are usually stopped two to three days before, and opioid painkillers are held. Do this only as your doctor advises.
Both give the same symptoms. In functional dyspepsia the emptying test is normal. The treatment is different, so the test matters.
Some cases, especially after a viral illness, improve over months. That is why a repeat test is worthwhile before any irreversible step.
