Rectal Cancer in the Elderly To Operate or Not to Operate A Nationwide Retrospective Study of the Italian Society of Collaborative Group.DOI 10.1097DCR.0000000000003961

Many people diagnosed with rectal cancer are over the age of 70. Doctors sometimes worry that surgery may be too risky for older patients. A large nationwide study from Italy looked at whether age alone should affect the decision to operate.

Researchers studied 3,573 patients who had rectal cancer surgery at 19 major hospitals. About 42% of the patients were aged 70 or older.
The results were reassuring. The risk of death within 90 days after surgery was very low in both groups—about 0.4% in younger patients and just over 1% in older patients. This difference was not considered significant.

Older patients did have more general medical complications after surgery and often needed closer monitoring in the intensive care unit. They also stayed in the hospital slightly longer than younger patients.
Despite this, the surgical outcomes and cancer control were similar between age groups.

Survival related specifically to the cancer was also comparable.Overall survival was lower in older patients mainly because they were more likely to have other health conditions unrelated to the cancer.

The study sends an important message: age alone should not prevent someone from receiving surgery for rectal cancer. With careful patient selection and proper care, many elderly patients can benefit from curative surgery.
