Medical Therapy for Acute Severe Ulcerative Colitis A Systematic Review With Meta analysis. DOI 10.1016j.cgh .2025.03.007

Ulcerative colitis (UC) is a chronic condition, but sometimes it can flare up suddenly and severely. This is called acute severe ulcerative colitis (ASUC). It can be dangerous and may lead to emergency surgery if not controlled quickly.

A new study reviewed 44 research papers to understand which medicines work best during these emergencies. It found that a drug called infliximab helped more patients avoid surgery than another drug called cyclosporine A. People treated with infliximab had a lower risk of needing to have their colon removed—at the time of hospital discharge and even up to a year later.

But infliximab also had a downside. Patients who took it were more likely to get infections compared to those who took cyclosporine A. So doctors have to weigh the benefits and risks carefully.

Another drug, tofacitinib, showed promise when used with steroids. It helped people feel better faster in the first week, but it didn’t lower the chance of surgery at 3 months.

These findings are important because they help doctors choose the right medicine during a crisis. More research is needed to find the safest and most effective treatments, especially for people who don’t respond to standard options.
